How We Collect Information About You: Orange County Sports and Wellness and its employees and volunteers collect data through a variety of means including but not necessarily limited to letters, phone calls, emails, voice mails, and from the submission of applications that is either required by law, or necessary to process applications or other requests for assistance through our organization.
What We Do Not Do With Your Information: Information about your financial situation and medical conditions and care that you provide to us in writing, via email, on the phone (including information left on voice mails), contained in or attached to applications, or directly or indirectly given to us, is held in strictest confidence. We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about applicants or clients who apply for or actually receive our services that is considered patient confidential, is restricted by law, or has been specifically restricted by a patient/ client in a signed HIPAA consent form.
How We Do Use Your Information: Information is only used as is reasonably necessary to process your application or to provide you with health or counseling services which may require communication between OCSW and health care providers, medical product or service providers, pharmacies, insurance companies, and other providers necessary to: verify your medical information is accurate; determine the type of medical supplies or any health care services you need including, but not limited to; or to obtain or purchase any type of medical supplies, devices, medications, insurance,
If you apply or attempt to apply to receive assistance through us and provide information with the intent or purpose of fraud or that results in either an actual crime of fraud for any reason including willful or un-willful acts of negligence whether intended or not, or in any way demonstrates or indicates attempted fraud, your non- medical information can be given to legal authorities including police, investigators, courts, and/or attorneys or other legal professionals, as well as any other information as permitted by law.
Information We Do Not Collect: We do not use cookies on our website to collect date from our site visitors. We do not collect information about site visitors except for one hit counter on the main index page (www.ocsportsandwlellness.com) that simply records the number of visitors and no other data. We do use some affiliate programs that may or may not capture traffic date through our site. To avoid potential data capture that you visited a diabetes website simply do not click on any of our outside affiliate links.
Limited Right to Use Non-Identifying Personal Information From Biographies, Letters, Notes, and Other Sources: Any pictures, stories, letters, biographies, correspondence, or thank you notes sent to us become the exclusive property of IHSN. We reserve the right to use non-identifying information about our clients (those who receive services or goods from or through us) for fundraising and promotional purposes that are directly related to our mission.
Clients will not be compensated for use of this information and no identifying information (photos, addresses, phone numbers, contact information, last names or uniquely identifiable names) will be used without client’s express advance permission. You may specifically request that NO information be used whatsoever for promotional purposes, but you must identify any requested restrictions in writing. We respect your right to privacy and assure you no identifying information or photos that you send to us will ever be publicly used without your direct or indirect consent.
You are in control of your information: You control who can access your personal health information. By default, you are the only user who can view and edit your information. If you choose to, you can share your information with others.
OCSW will not sell, rent, or share your information (identified or de-identified) without your explicit consent, except in the limited situations described in the detailed OCSW Privacy Policy, such as when OCSW believes it is required to do so by law.
You can completely delete your information at any time. Such deletions will take immediate effect in your account, and backup copies of deleted information may persist for a short time. Since deleted data will not be restored, you may want to print information before deleting it.
How OCSW uses your information: To store your information in OCSW, you will need a OCSW Account. When you create a OCSW Account, OCSW asks for your email address and a password, which is used to protect your account from unauthorized access.
OCSW servers automatically record log information about your use of OCSW (such as number of sign-ins and number of times a link was clicked). This information is temporarily stored in association with your OCSW Account and log information will be used to operate and improve the service.
OCSW will use aggregate data to publish trend statistics and associations. None of this data can be used to personally identify an individual.
Certain features of OCSW can be used in conjunction with other OCSW products and services, and those features may share information to provide a better user experience and to improve the quality of our services.
Sharing your information with people and services you trust: OCSW works with third-party that are capable of securely sending information to OCSW. These websites (which may include your medical provider) may give more information about certain conditions or extend the functionality of OCSW in other ways. By creating a link to these third parties, you give them permission to send you information such as medical records, prescription histories, or test reports.
You can approve access for some of these third-parties to view your health information. If a third-party accesses your health information and stores a copy of your information, that copy will be governed by the privacy policies of third-parties. Others at the third-party may be able to view your information. OCSW is not responsible for the content, performance, or privacy policies of third-parties.
If you share your information with others, you can view a list of who has access to your information and you can revoke sharing privileges at any time. When you revoke someone's access to information, that party will no longer be able to read your information, but may have already seen or may retain a copy of the information.
Some of these third-parties will be covered by federal and state health privacy laws (such as the Health Insurance Portability and Accountability Act, or "HIPAA"), and those laws will govern how they may use and share your information. As a result, you must authorize these providers to send information to your OCSW account. With that authorization, you also give them permission to send certain types of health information (such as mental health records) that are protected by federal and state laws and require special authorization. When you ask OCSW to send your health information to others, you will also be giving Crossover permission to send those certain types of health information.
All third-party partners of OCSW are contractually required to abide by the OCSW Privacy Policies, which establish strict privacy standards for how they collect, use, or share your information.
